5 stars: For all the comments below, some are unjustified (like not wanting to use Itsme), and many are outdated. Chrome is not required, login via CSAM (hence many options) is now possible. One small glitch: CSAM in Dutch when English selected. And the app does not require your location etc. It does what it needs to do: just show the QR code.
4 stars: Yes, Chrome IS required, and by required it means installed, it opens in Firefox (default browser for me) after you installed Chrome. (Kinda dumb but hey....) Used Ehealth to log in with the one time password via mail, redirected me to the app and all done, two certificates.

4 stars: The app works fine, but why is there a giant Delete button at the bottom of the certificates? That's just waiting to be pressed accidentally. If it is necessary to have that, you should make it smaller and tuck it away in a corner or have it only accessable through a menu.
3 stars: It's not at all understandable that you have to click on the vaccination info in order to see the qr code. It would be so much easier if there was a link saying something like 'Show QR code'
4 stars: During the installation of the app any problems but now the expiration date of the vaccination is no longer visible. Can you fix this problem please? Thank you!
